- You will pay the participating Aetna Dental Access® network provider the discounted fees at the time of service.
- There are no claim forms to submit for reimbursement.

Outlook Vision Services
- Select a participating provider - Find a vision provider
- Your ID card showing the Outlook logo is all you need to use your Outlook discount
benefit.
- Simply take your current vision prescription to any Outlook provider
to purchase your new eyewear and pay the discounted amount directly to the
provider at the time of purchase. A nominal dispensing fee may apply in certain
areas.
