Benefit Description


All vision services offer by an authorized provider in a participating network are eligible for discounts ranging from 5% to 50% under the plan. No vision services offered by a participating provider for which no other discount is available are excluded.
 
When visiting a participating vision services location, present you membership card prior to treatment to be assured that the proper discount is applied. Payment for services is due at the time they are rendered. Provider lists and fees may change at any time.
 
The Access One Consumer Health Vision Service Plan provides discounts on the following services at participating providers:
 
  • Prescription Lenses and Frames
  • Ophthalmologic Services including LASIK, RK, and CO2 Laser Surgery
  • Contact Lenses and Sunglasses
