1. Select a participating provider from the Dental Provider Lookup
2. Let the provider know that you are part of the Aetna Dental Access® network.
3. You will pay the participating Aetna Dental Access® network provider the discounted fees at the time of service.
4. There are no claim forms to submit for reimbursement.

Outlook Vision Services

1. Select a participating provider – Find a vision provider
2. Your ID card showing the Outlook logo is all you need to use your Outlook discount benefit.
3. Simply take your current vision prescription to any Outlook provider to purchase your new eyewear and pay the discounted amount directly to the provider at the time of purchase. A nominal dispensing fee may apply in certain areas.
4. Mail-order contact lenses also available at 1-800-CONTACTS.
